Predictable, transparent pricing for routine visits, sick visits, chronic disease management, and annual wellness care — for patients ages 13 and up.

Individual membership is $50/month (current introductory rate) plus $25 per visit, compared with $99 per visit without membership. The $50/month rate is introductory — it will increase to $69/month once MedFIT reaches its initial membership enrollment goal.

If you enroll now, you keep your $50/month rate for as long as your membership stays continuously active and payments stay current — this isn't a fixed-term contract. Re-enrolling after a lapse or cancellation would be at the then-current rate.

Membership Terms & Agreement

Important Membership Disclosure

The MedFIT Primary Care Membership is a direct primary care membership and is not health insurance. It does not replace health insurance coverage for hospital care, emergency services, specialists, prescription medications, imaging, outside laboratory testing, or other services not specifically included in the membership.

By enrolling in a MedFIT Primary Care Membership, the member agrees to the pricing, eligibility requirements, visit limits, payment terms, exclusions, and other conditions described below.

Membership is available for adults and eligible dependent children age 13 and older.

Membership Plans & Pricing

Individual Membership

$69 per month

Introductory individual rate: $50 per month

Provider visits: $25 per visit

Couple Membership

$119 per month

Includes two enrolled adults.

Provider visits: $25 per visit, per person

Family Membership

$159 per month

Includes two adults and up to two eligible dependent children age 13 or older.

Additional eligible children may be added for $20 per month each.

Provider visits: $25 per visit, per person

Employer Membership

Available to businesses with 5 or more enrolled employees.

Employee only: $49 per month
Employee + spouse: $99 per month
Family: $139 per month
Additional eligible child age 13+: $20 per month

Provider visits: $25 per visit, per enrolled person

Employer family pricing follows the same spouse, family, and additional-child pricing increases used for standard MedFIT memberships.

Visit Allowance & Non-Transferable Visits

Each enrolled member has access to up to 13 provider visits per membership year at the standard member visit rate.

The visit allowance belongs only to the individual enrolled member.

Visits cannot be transferred, shared, traded, pooled, or used by another family member, employee, spouse, dependent, or non-member.

Unused visits do not create a credit or cash value and cannot be carried over to another person.

If an enrolled member reaches their own annual 13-visit allowance, additional visits may still be available at a reduced member cash rate.

The annual visit allowance resets according to the member’s membership year.

What the Membership Includes

The MedFIT Primary Care Membership is designed to provide access to routine primary care services that can be appropriately delivered by MedFIT.

Eligible services may include:

  • Sick visits

  • Routine primary care visits

  • Chronic condition follow-up

  • Medication management

  • Blood pressure management

  • Diabetes care

  • Thyroid care

  • Preventive and wellness visits

  • Birth control pill management when medically appropriate

  • Routine women’s primary care

  • Evaluation of minor medical concerns

  • Referrals and care coordination when appropriate

The exact care provided at any visit depends on the member’s medical needs, the clinical judgment of the treating provider, and whether the service can be safely and appropriately provided by MedFIT.

Testing, Labs, Imaging & Additional Charges

Some visits require testing or services that carry a separate direct cost and are not included in the monthly membership fee or the $25 provider visit fee.

These may include:

  • Flu testing

  • Strep testing

  • COVID testing

  • Urinalysis

  • Pregnancy testing

  • Other available in-office rapid tests

  • Outside laboratory testing

  • X-rays

  • Ultrasounds

  • CT scans

  • MRIs

  • Prescription medications

  • Certain vaccines, injections, medications, or supplies purchased by the clinic

  • Specialist services

  • Hospital or emergency-room care

  • Other third-party services

When possible, MedFIT will inform the member of known additional in-office charges before the service is performed.

Charges from outside laboratories, imaging centers, pharmacies, specialists, hospitals, or other third parties are billed separately by those providers and are the responsibility of the member.

Services Not Included in Membership

The MedFIT Primary Care Membership does not include services outside the scope of routine primary care or services offered through separate MedFIT programs.

Examples include:

  • Medical Weight Loss services or medications
  • Testosterone or other hormone evaluation and treatment
  • Aesthetic services
  • Pelvic floor therapy
  • Workers’ compensation care
  • Motor vehicle accident or legal-case treatment
  • Emergency care
  • Hospital care
  • Specialist care
  • Major procedures
  • Outside laboratory or imaging services
  • Prescription medications
  • Services requiring equipment, supplies, medications, or third-party charges not included in the membership

Separate MedFIT services may be available at their standard pricing.

Membership does not guarantee that every medical condition can be treated by MedFIT. When a member needs care outside MedFIT’s scope, the provider may recommend referral to a specialist, urgent care center, emergency department, hospital, or other appropriate medical resource.

Payment, Renewal & Failed Payments

Membership fees are billed on a recurring basis according to the payment method selected at enrollment.

Membership continues automatically unless canceled in accordance with the cancellation terms below.

Members are responsible for maintaining a valid payment method.

If a scheduled membership payment is declined, returned, or otherwise unsuccessful, MedFIT may suspend membership benefits until the account is brought current.

During any suspension for nonpayment:

  • The member may lose access to member pricing
  • Provider visits may be charged at the standard cash-pay rate
  • Membership benefits may remain unavailable until payment is successfully processed

MedFIT may attempt to process an outstanding payment again using the payment method on file.

Membership fees already paid are generally not refundable simply because the member did not use available visits or services during that billing period.

MedFIT may update membership pricing in the future. Members will be given notice before any new recurring membership rate takes effect.

Cancellation & Termination

Members may cancel their MedFIT Primary Care Membership by providing written notice to MedFIT.

Cancellation stops future recurring membership charges after the applicable cancellation date. Membership benefits remain subject to the member’s current paid membership period and account status.

Membership fees already paid are generally not refundable or prorated simply because the member chooses to cancel before the end of a paid billing period.

MedFIT may terminate or suspend a membership for reasons that may include:

  • Nonpayment
  • Fraud or misuse of membership benefits
  • Sharing or attempting to transfer membership benefits
  • Abusive, threatening, or disruptive behavior
  • Repeated failure to follow clinic policies
  • Circumstances in which MedFIT determines it can no longer safely or appropriately provide ongoing primary care

If MedFIT terminates a membership for reasons unrelated to member misconduct or nonpayment, MedFIT will make reasonable efforts to provide appropriate notice and transition information when clinically necessary.

Cancellation or termination of membership does not eliminate responsibility for unpaid balances or charges incurred before the membership ended.

Employer Membership Rules

Employer membership pricing is available to businesses with 5 or more enrolled employees.

Current employer membership pricing is:

  • Employee only: $49 per month
  • Employee + spouse: $99 per month
  • Family: $139 per month
  • Additional eligible child age 13 or older: $20 per month
  • Provider visits: $25 per enrolled person, per visit

Family enrollment includes the employee, spouse, and up to two eligible dependent children age 13 or older.

Each enrolled family member receives their own individual annual visit allowance. Visits cannot be shared, pooled, transferred, or reassigned between enrolled family members.

Employers and enrolled employees are responsible for providing accurate enrollment information and notifying MedFIT when an employee or dependent should be added or removed.

Employer payment arrangements may vary by participating business. Depending on the arrangement, the employer may pay all or part of the membership fee, or the employee may be responsible for some or all of the cost.

The MedFIT Employer Membership is a primary care membership and is not health insurance. It does not replace coverage for hospital care, emergency services, specialists, prescription medications, imaging, outside laboratory testing, or other services not included in the membership.

If an employee leaves a participating employer, changes eligibility status, or is removed from the employer plan, employer membership pricing may end. The individual may contact MedFIT about available individual or family membership options.

Emergency Care & Medical Limitations

The MedFIT Primary Care Membership is intended for routine primary care and appropriate non-emergency medical needs.

It is not an emergency medical service.

If a member is experiencing a medical emergency or symptoms that may require immediate evaluation, the member should call 911 or go to the nearest emergency department.

Examples of symptoms that may require emergency care include:

  • Chest pain
  • Severe shortness of breath
  • Signs of stroke
  • Severe bleeding
  • Loss of consciousness
  • Severe allergic reaction
  • Serious injury
  • Other potentially life-threatening symptoms

MedFIT providers may determine that a condition cannot be safely or appropriately managed within the clinic and may recommend urgent care, emergency evaluation, specialist care, hospital services, or another appropriate level of care.

Membership does not guarantee access to a specific provider, same-day appointment, diagnosis, treatment, medication, referral, or particular medical outcome.

Not Insurance + Agreement & Acceptance

The MedFIT Primary Care Membership is a direct primary care membership and is not health insurance.

Membership does not provide insurance coverage for services such as:

  • Hospital care
  • Emergency services
  • Specialist care
  • Prescription medications
  • Imaging
  • Outside laboratory testing
  • Major procedures
  • Other services not specifically included in the membership

Members are responsible for obtaining and maintaining any health insurance coverage they believe is appropriate for services outside the scope of the MedFIT Primary Care Membership.

By enrolling in the membership, the member acknowledges that they have reviewed and agree to the membership pricing, eligibility requirements, visit limits, exclusions, payment terms, cancellation terms, and other conditions described in this agreement.

Enrollment in a couple, family, or employer plan does not combine or pool visit allowances. Each enrolled person has their own individual membership benefits and visit limits.

MedFIT may update these Membership Terms & Agreement from time to time. Material changes affecting pricing or recurring membership terms will be communicated before they take effect.
